Prevention of Saphenous Nerve Injury after Endovenous Laser Ablation of Incompetent Great Saphenous Veins: 2 Step Ablation

چکیده
血管内レーザー焼灼術(EVLA)後に生じる伏在神経障害を回避する手技について前向きに検討した.下腿まで逆流が認められる大伏在静脈(GSV)弁不全72例90肢に対しEVLAを施行した.膝上GSVは出力7 W(目標LEED 50~70 J/cm)で焼灼し,膝下GSVでは出力を5 20~25 J/cm)に落として,条件を変えて分割焼灼した.GSVの平均焼灼長は50.4 cmであった.術後1カ月目の超音波検査では全例において内腔閉塞が確認され,神経障害はみられず,根治性の高いEVLAが可能であった.
